REQUIRED DOCUMENTATION

that may help support a VA Service-Connected Disability claim

- Discharge Document (DD Form 214 or WD AGO Form)

  • Service Medical Treatment Records (Outpatient), dental records, entrance and separation examination reports
  • Inpatient Hospitalization Records (private / military)
  • Private Outpatient Medical Records
  • Personnel File
  • Line of Duty (LOD) investigation reports
  • Overseas or Temporary Duty (Travel) orders

 

         Separating personnel should ensure that all of the information (including awards, overseas time, campaigns, RE code, etc.) on the DD 214 is correct before signing.  Uncorrected errors may cause problems in the future when applying for VA claims and other benefits.

 

-  Martial History:

  • Marriage Certificate (Current)
  • Divorce Decree(s) (for each previous Marriage)
  • Death Certficate (for each previous Marriage)

-  Birth Certificate(s) & SSAN (For each Dependent under Age 18, if over 18 and still in school: will need a letter of verification from school / college)

-  Court Order (Child Support; adoption, etc.)

-  Financial Information (Bank Routing / Checking Account Number)
